Boys Youth Speed & Agility Clinic
WAIVER OF LIABILITY
Please read this waiver carefully in its entirety. By signing below, the parent or legal guardian acknowledges and agrees to all terms on behalf of the minor participant.
1. Assumption of Risk
I, the undersigned parent or legal guardian, acknowledge that participation in the Boys Youth Speed & Agility Clinic (the "Event") involves physical activity including, but not limited to, running, jumping, agility drills, resistance training, and other athletic exercises. I understand that such activities carry inherent risks of injury, including but not limited to sprains, strains, fractures, and other physical harm. I voluntarily assume all such risks on behalf of the minor participant named below.
2. Release of Liability
In consideration of my child's participation in the Event, I hereby release, waive, discharge, and covenant not to sue Home Team Empowerment Center, Next Level Athletic Fitness, Alpha Settlement, their respective officers, directors, employees, volunteers, agents, and representatives (collectively, the "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by the minor participant during or in connection with the Event, whether caused by the negligence of the Released Parties or otherwise.
3. Indemnification
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur due to the participation of the minor in the Event, whether caused by the negligence of the Released Parties or otherwise.
4. Medical Authorization
I authorize the Event staff to seek emergency medical treatment for the minor participant if I cannot be reached in a timely manner. I accept full financial responsibility for any medical treatment rendered. I certify that the minor participant is physically fit to participate in athletic activities and has no medical conditions that would prevent safe participation, or that I have disclosed all relevant medical conditions in the registration form.
5. Photo & Media Release
I grant permission to Home Team Empowerment Center and Next Level Athletic Fitness to photograph and/or video record the minor participant during the Event and to use such images or recordings for promotional, educational, and social media purposes without compensation.
6. Code of Conduct
I agree that the minor participant will conduct themselves in a respectful and sportsmanlike manner throughout the Event. I understand that disruptive, dangerous, or disrespectful behavior may result in removal from the Event without refund.
7. Severability
If any provision of this waiver is found to be unenforceable or invalid, that provision shall be limited or eliminated to the minimum extent necessary so that this waiver shall otherwise remain in full force and effect and enforceable.
Acknowledgment & Signature
By signing below, I confirm that I have read and fully understand this Waiver of Liability, that I am the parent or legal guardian of the minor participant, and that I am signing this waiver freely and voluntarily.
